David, The chapters available on-line can simply be used with my permission, which you now have. They are here at no cost and the permission is simply a safety for me, so no one else can "publish" them. However, they are so old (1986-7 or so), that they are not in latex, and I have never had the time to convert them to latex. (Converting the text would be easy, but the diagrams would all have to be done over and that has always stopped me.) I did do a bit of modification to them about 5 years ago, but in general they are fundamentally the original book. So go ahead and use them as you see fit.
